Description As a Hospice Patient Care Coordinator (RN) you will be called to care when you're needed most. As part of Interim HealthCare, you'll provide oversight of the care/services furnished by employees to Interim HealthCare Hospice patients. As a Hospice Patient Care Coordinator (RN), you will: Collaborate with the Director of Clinical Services to develop and implement processes to ensure the efficient and effective achievement of patient/client outcomes in accordance with physician ordersProvide ongoing education to hospice employees including, but not limited to: regulations, guidelines, current standards of hospice and palliative care practice, Interim HealthCare Hospice polices and procedures, quality assessment and performance improvement (QAPI) programs, and each employee's role in eachSupervise, evaluate, motivate, and develop professional and paraprofessional employeesUse clinical decision making to provide oversight and guidance to the healthcare team to efficiently and effectively manage the assigned patient populationServe as an expert for the hospice admission process and perform admissions as assignedTo qualify for a Hospice Patient Care Coordinator (RN) with us, you will need: Graduate of an accredited school of nursing and licensed to practice as an RN in the state in which employed.
