We are so glad you are interested in joining Sutter Health! **Organization:** PAMF-Palo Alto Medical Foundation PAD **Position Overview:** ** This position will support the Bay Area and is eligible to work from home but must be available to go on-site when needed ** You will be working in the Sutter Sync program, which is a comprehensive care management program designed to virtually manage patients with chronic conditions such as hypertension, diabetes, and other chronic disease states using remote patient monitoring tools and other various pieces of technology. Pharmacists will manage a panel of patients in collaboration with a team of Health Coaches and can adjust medications or offer personalized lifestyle advice to patients from the comfort of their home. We expect a heavy onsite presence at our Innovation Center in San Francisco for the first few weeks or months of the project. After we are stable, there will be an opportunity to work remotely, though additional onsite time may be required as we add new conditions. This role will help start up a new clinic within Sutter Health, with an expectation that you will help shape the final pharmacist workflows and assist with creating educational and onboarding materials/workflows for additional pharmacist hires for the program. Oversees and delivers pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost efficient medication therapy. Exercises sound judgment, and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. May also be responsible for performing specific procedures and/or teaching duties.
